A named GP
should take overall responsibility for a patient who is dying in the
community, the team behind the Liverpool Care Pathway review
has recommended. They say that it is “particularly good practice” to
give dying patients a telephone number on which they can reach their GP
if there is an out-of-hours emergency.
The review authors also
said that the patient’s GP should be involved in discussing their care
plan if they have no family or carers.

Neighbourhood watch groups should provide care to 'miserable and lonely' pensioners, says health minister
Norman Lamb warned that Britain’s attitude to the elderly was “uncivilised”
and said the public must acknowledge that pensioners living nearby need
their help.
He wants neighbourhood watch groups to apply for “care status” and suggested
that they could help wash and feed pensioners as well as provide
companionship.
The plans, which have the approval of senior Government figures and have been
welcomed by Neighbourhood Watch leaders, would help to reduce the number of
pensioners forced into care homes, the Liberal Democrat minister hopes.

Hospitals must improve links with dementia carers, study finds
Hospitals are failing to link up sufficiently with the carers of
patients suffering from dementia, an audit carried out by the Royal
College of Psychiatrists has found.
Despite improvements over the last two years, a third of hospitals still do not have guidance in place on the involvement of and information sharing with the person’s carer, according to the latest National Audit of Dementia.
The audit, the first of which was carried out in 2011, found case notes often do not include information that could aid communication with carers, while information important to future care is not routinely summarised at the point of discharge.

People with learning disabilities still face unacceptable inequalities in healthcare

Two new publications from the Department of Health, the responses to the Confidential Inquiry into premature deaths of people with learning disabilities and the Six Lives Progress Report on Healthcare for People with Learning Disabilities, show that whilst some improvements have been made, people with learning disabilities are still experiencing poor care, and face unacceptable inequalities in health and social care.
