Royal College of General Practitioners (RCGP) – CARING ABOUT CARERS AWARDS

Do you look after a relative or friend who is sick, disabled, frail or has a problem with alcohol and/or drugs?

IF YES… then you know how help from your GP and their practice can make all the difference.

The Royal College of General Practitioners, in partnership with The Carers Trust, are looking to find GPs and/or Practice Teams who provide the best support to unpaid carers.

Nominate your GP or Practice Team for this special award by telling us how they support you in your caring role. To obtain a nomination form either email: scourt@rcgp.org.uk or telephone on 029 20 504854.

1. your name, address and telephone number
2. your Doctor’s name or Practice Team’s contact details
3. how they support you in your caring role
