SURVEY: Local rheumatology services in BaNES and Wiltshire – online survey for patients and carers



The Rheumatology Commissioning Support Alliance (RCSA) is trying to gain a better insight into the local rheumatology services in Bath and North East Somerset and Wiltshire from the perspective of local patients with rheumatological conditions and their carers.

This information will be shared with local commissioners in order to provide options on how to improve the service. The information will also help to inform a specially convened stakeholder meeting on 11th September.

There are many different rheumatological conditions including lower back pain, ankylosing spondylitis, lupus, osteoarthritis, psoriatic arthritis, or rheumatoid arthritis. Most of the questionnaire relates to health and social care services but some issues will cross into other areas e.g. housing.

If you have more than one rheumatological condition and you feel the questionnaire doesn’t let you answer the questions about both of your conditions at the same time, please choose one condition to use in your answers, or please do more than one online survey.

If you are a patient that uses these local services in these areas then we would be very grateful if you would please complete the following questions in full, which should take no more than 15 minutes to complete.

Please rest assured that all results will be added up and made anonymous. Comments will not be attributed to any individuals as we want to encourage as much honest feedback as possible in order to help improve local services.

The survey closes at midnight on Sunday, 08 September 2013.
